Please complete ‘Patient Registration‘ to send information directly to WHD. If you prefer to print out forms, please click ‘Print Patient Forms‘ and bring the completed forms to your appointment 15 minutes early. If you are having Patch testing, please click ‘Print Patch Testing Forms‘, fill them out and bring them with you. Please note that Patch Testing patients must complete both the ‘Patient Registration‘, and the ‘Patch Testing Forms‘.
